Yes. When appropriate, our doctors can provide notes for your workplace or school, limited to short-term illness or non-work-related injury. Depending on your evaluation, we can also provide documentation for workplace needs such as sit-stand desks and ergonomic chairs. Please note that we cannot complete work injury-related documentation, Family and Medical Leave Act (FMLA), Disability paperwork or other similar documentation.
Begin by starting a visit and having one of our physicians assess your condition. Depending on your diagnosis, as well as our clinic’s guidelines, we’ll send you a doctor’s note via email through our secure Zendesk portal.
Unfortunately, we are not able to provide accommodation notes that require in-person evaluations or notes that are dated prior to your 98point6 visit.
